Skilled In-Home Physical Therapy (Medicare Part B)

Medically necessary physical therapy delivered in the home under a physician’s plan of care.

Non-Clinical Mobility Assessments

Fall Risk Assessment & Prevention

A one-time, non-clinical in-home assessment focused on identifying fall risk and safety concerns — without automatically starting therapy.

Post-Discharge Mobility Assessment

A short-term in-home mobility and safety assessment completed after hospital or rehab discharge.

Community Education & Outreach

Southern Steps also provides community outreach and education, including fall-prevention talks and caregiver workshops. These offerings are educational only and separate from individualized care.

Do I need a doctor’s referral to start?

Only skilled physical therapy requires a physician referral. Non-skilled assessments and maintenance services do not.
