A time-limited, non-clinical in-home mobility and safety assessment completed shortly after discharge, focusing on:

→ Functional readiness

→ Mobility confidence

This assessment does not automatically start physical therapy.

→ Recently discharged hospital or rehab patients

→ Individuals returning home after skilled care

→ Families seeking reassurance during transitions

Who This Is Not For

→ Ongoing therapy treatment

→ Skilled rehabilitation services

→ Long-term care management

What Happens After the Assessment

Based on findings, individuals may:

→ Some transition into non-skilled support

Be guided toward physical therapy if medically necessary

When should this assessment be scheduled?

Ideally within the first week after discharge.

Is this the same as home health therapy?

No. This is a non-skilled assessment, not treatment.

Does this require a doctor’s order?

No physician referral is required.

Can this prevent readmissions?

The goal is to identify risks early and clarify next steps before problems escalate.
