Most people assume you need a doctor's referral and insurance approval before you can start physical therapy at home. That's not always true.

I'm Mike Zagare, a licensed physical therapist with Southern Steps PT in Greenville, SC. About a third of my patients pay out of pocket — what we call private pay or self-pay — and they often tell me they wish they'd known it was an option sooner.

So let me walk you through exactly how private pay in-home physical therapy works, what it costs, and why some patients actually prefer it over going through insurance.

What Is Private Pay Physical Therapy?

Private pay simply means you're paying for physical therapy directly, without billing insurance. You pay the therapist, the therapist treats you. That's it.

There's no middleman deciding how many visits you get. No pre-authorization paperwork. No waiting two weeks for an approval that might get denied anyway.

You might also hear it called self-pay, cash pay, or out-of-pocket physical therapy. They all mean the same thing.

Why Would Someone Choose Private Pay Over Insurance?

This is the question I get most often. If insurance covers PT, why would anyone pay out of pocket?

Here are the real reasons my private pay patients give me:

They don't want to wait. Medicare and insurance require a doctor's referral, then pre-authorization, then scheduling. That process can take days or weeks. With private pay, I can often see someone within 48 hours of their first phone call.

They want more control over their care. Insurance companies decide how many visits you're approved for — usually a set number that may or may not be enough. With private pay, we keep going until you've met your goals. If you need 6 visits, great. If you need 15, that's fine too. We decide together based on how you're progressing, not based on what an insurance reviewer thinks from behind a desk.

They want to skip the paperwork. Medicare does cover in-home physical therapy — under Part A (home health) if you're homebound, and under Part B (outpatient) even if you're not. But both require a doctor's order, and Part A requires a face-to-face visit and a home health certification. Some patients just don't want to deal with any of that. Private pay cuts out the paperwork entirely.

They want privacy. Some people simply don't want their insurance company involved in their healthcare decisions. Private pay keeps it between you and your therapist.

They don't have insurance that covers in-home PT. Not every plan covers physical therapy at home. Some plans only cover clinic visits. Private pay means you're not limited by what your plan does or doesn't include.

What Does Private Pay In-Home Physical Therapy Cost in Greenville?

I believe in being upfront about money. Nobody likes calling a healthcare provider and getting the runaround on pricing.

At Southern Steps, private pay sessions for in-home physical therapy typically range from$150 to $200 per visit. The exact rate depends on the complexity of your condition and the length of the session.

Here's what that includes:

  • A licensed physical therapist comes to your home — no clinic visit, no driving

  • One-on-one treatment for the full session (not 15 minutes with a PT and 45 minutes with an aide, like you might get at a clinic)

  • A personalized treatment plan designed around your home environment

  • Exercise programs you can do between visits

  • Direct phone and text access to your therapist for questions between sessions

For context, a typical course of in-home PT runs 6 to 12 visits over 4 to 8 weeks. So you're looking at roughly $900 to $2,400 total for a full course of treatment. Some patients need fewer visits, some need more — we'll know a lot more after the initial evaluation.

Private Pay vs. Medicare: Which Makes Sense for You?

Neither option is better across the board. It depends on your situation.

Medicare might be the right choice if:

  • Your doctor agrees you need in-home physical therapy

  • You don't mind the referral and authorization process

  • You want to minimize out-of-pocket costs (most Medicare patients pay $0 under Part A, or 20% coinsurance under Part B)

  • You qualify for home health (Part A) or outpatient PT (Part B)

Private pay might be the right choice if:

  • You want to start right away without waiting for a doctor's order

  • You want full control over how many sessions you receive

  • Your insurance doesn't cover in-home PT

  • You value privacy and simplicity

And here's something most people don't realize:you can use both. Some of my patients start with private pay to get going immediately, and then we transition to Medicare once their referral and authorization come through. Others use Medicare for their initial course of care and then continue with private pay after Medicare's approved visits run out.

What Conditions Does Private Pay In-Home PT Treat?

The same conditions I treat through Medicare. The difference isn't what we treat — it's how we pay for it. Common reasons my private pay patients come to me:

  • Recovery after hip or knee replacement surgery

  • Balance problems and fall prevention

  • General weakness and deconditioning (especially after a hospital stay)

  • Back pain and neck pain

  • Stroke recovery

  • Difficulty walking or getting around the house safely

  • Arthritis pain and stiffness

  • Post-surgical rehabilitation of any kind

If you're dealing with something that affects your ability to move safely and comfortably at home, in-home PT can probably help — regardless of how you pay for it.

How to Get Started with Private Pay In-Home PT

This is the part people are usually surprised by. It's simple:

That's it. No referral. No paperwork to submit to insurance. No waiting for approval.

If at any point we determine that you might qualify for Medicare coverage, I'll let you know and we can explore that option together. I'm not going to keep you on private pay if insurance would cover you — that's not how I operate.

Frequently Asked Questions About Private Pay PT

Do I need a doctor's referral for private pay physical therapy?
No. South Carolina allows direct access to physical therapy, which means you can see a PT without a referral. For private pay patients, you can call me directly and we'll get started.

Can I use my HSA or FSA to pay for private pay PT?
In most cases, yes. Physical therapy is generally an eligible expense for Health Savings Accounts and Flexible Spending Accounts. Check with your plan administrator to confirm.

Do you offer any payment plans?
We can discuss payment arrangements on a case-by-case basis. My goal is to make sure cost isn't the reason someone doesn't get the care they need.

What if I have Medicare but don't want to use it?
That's completely fine. Some patients prefer private pay even when they have Medicare coverage. It's your choice.

How many sessions will I need?
It varies. Some patients see significant improvement in 4 to 6 sessions. Others with more complex conditions might need 12 or more. We'll talk about expectations during your first visit, and I'll give you an honest estimate — not a sales pitch.
